As an essential business, employees of Toyoda Gosei (Missouri) are being evaluated to receive the vaccine on site in the coming weeks by the state of Missouri. No exact date(s) has been given. However, in order to better plan for this, the Health Department is asking for specific numbers to assist with vaccination ordering and allocation as doses are limited at this time and may continue to be limited. As an employer, we value your health so that first you can stay healthy and working to provide for your family. Second, TG can in turn meet the needs of our customers. Therefore, the purpose of this survey is three fold:

  1. Announce the upcoming possibility of vaccines offered on site.
  2. Encourage our team members to take the vaccine.
  3. Obtain an exact count on total requests for the vaccine for state and local health dept. planning purposes.

8. Do you wish to receive the COVID-19 vaccine, understanding and agreeing to take the second booster dose 28 days later?

FOLLOW UP INFORMATION: On the date the vaccine is given, you will be asked to fill out a health questionnaire.
A) Based on the questionnaire, in some cases, the health experts may determine you should not receive the vaccine.
B) You will be evaluated for a minimum of 15 minutes following administration of the vaccine to ensure there is no extreme adverse reaction (rare), for which the health experts on site are fully prepared to address.
C) The following are all typical responses for the body building an immune response to COVID-19 following taking the COVID-19 vaccine: fever, fatigue, headache, chills, muscle aches, tenderness at injection site, nausea. These are all typical responses for the body building an immune response to COVID-19.
D) You must agree to get the booster 28 days after the initial injection is given, and you must bring back your vaccination record card at the time of the booster.
